You may obtain a certified copy of a birth certificate in person (with valid ID) at our office or through the mail by completing and mailing this form.  If you are requesting a birth certificate of a deceased person, complete and mail this form along with a copy of your driver's license or acceptable identification. The cost is $20 for the first certified copy and $6 for each additional copy of the same record.

 

*Because of the confidential nature of these records, Illinois Statutes provide that copies of birth records may only be obtained by the person themselves (if 18 years old), a parent, or other legal agent.

 

 

You may obtain a certified copy of a death certificate in person at our office or through the mail by completing and mailing this form along with a copy of your driver's license or acceptable identification. The cost is $20 for the first certified copy and $10 for each additional copy of the same record. The cost for genealogy purposes is $20 for the first copy and $6 for each additional copy of the same record.
